Business Service Agreement Terms

By submitting this application, you agree to the following:

1. Service Commitment: Services provided by Warm Springs Telecommunications Company (WSTC) are subject to availability and technical feasibility at the requested service address.

2. Credit Authorization: I authorize WSTC to obtain business credit reports and verify other information to determine eligibility for service. I understand that a deposit may be required based on credit history.

3. Installation: Professional installation will be scheduled upon approval of this application. Installation fees may apply. Customer is responsible for ensuring access to the property and providing any necessary property owner permissions.

4. Service Terms: Service is provided according to WSTC's Business Service Agreement, Terms of Service, and Acceptable Use Policy available at www.wstel.com. Monthly charges apply after installation.

5. Equipment: WSTC equipment must be returned upon service termination. Customer is responsible for any unreturned or damaged equipment.

6. Cancellation: Early termination fees may apply. Please refer to the complete Business Service Agreement for details.

7. Privacy: Your information will be used in accordance with WSTC's Privacy Policy and will not be sold to third parties.

8. Accuracy & Authority: I certify that all information provided in this application is true and accurate to the best of my knowledge, and that I am authorized to enter into this agreement on behalf of the business entity named above.
